Der Patient will gesund werden oder wenigstens so gut wie maglich mit seiner Krankheit leben. Eine noch so genaue Bezeichnung seiner Krank- heit allein nutzt ihm nichts. Die Diagnose hat ihr Ziel verfehlt, wenn sie lediglich den Patienten als Diabetiker, Herz-oder Niereninsuffizienten, Ubergewichtigen, Leukamiker oder Asthmatiker bezeichnet. Die Diagnostik soll erkennen lassen, wie der Patient zu seiner Krankheit gekommen ist und was diese Krankheit fUr den Patienten bedeutet. Nicht das jetzige Ergebnis, sondern die Prozesse, die dazu gefiihrt haben, sind zu analysieren; warum, wann und wo Regelprozesse gestort wurden oder versagt haben. Die Diagno- stik soll maglichst auf molekular-biologischer Ebene die individuellen Vari- anten der Lebensprozesse erkennen lassen. Das Resultat soll nicht die Krankheitsbezeichnung, sondern der Behandlungsplan sein. Vergleichen wir die Klinische Chemie von heute mit diesen uns allen be- kannten Forderungen, so mUssen wir feststellen, daB die Klinische Chemie Uber weite Strecken nur phanomenologische Informationen liefert, uns nur Uber Spuren abgelaufener Prozesse, nicht jedoch Uber den zugrundeliegenden KrankheitsprozeB selbst informiert. Der Blutzuckerspiegel z. B. laBt nicht erkennen, worin die Storung der Blutzuckerregulation im Einzelnen liegt. Genauso wenig kann die Verteilung der Blut-EiweiB-Fraktionen im Elektro- pherogramm erklaren, wie eine beobachtete Umverteilung zustandekommt.
