Ready to use legal forms for North Carolina in the book or free online are in 3 main areas: - - - - WILL RELATED FORMS - - - - Form 1. Will (Standard) - a Will (also called a "Last Will And Testament") lets a person control things after their death like who gets their money and property, who is Executor, and if easier legal options are OK later. Form 2. Will (Guardian) - this is a Will with a part added to name a person to be Guardian to care for a minor child under 18 if needed (like if both parents later die) and also manage a child's property and money. Form 3. Self-Proving Affidavit - optional form done with a Will to later help use a Will after a death. Form 4. Handwritten Will - this Will skips the usual 2 witnesses which saves some work, but all of it must be handwritten by the person doing the Will. - - - - HEALTH CARE FORMS - - - - Form 5. Health Care Power Of Attorney - lets a person name someone to control health care if later needed (like due to a person's later incapacity from inability to talk) and also write health care instructions. Form 6. Advance Directive For A Natural Death ("Living Will") - this form does serious act of saying stop most health care if later a person is incapacitated and doctors think the health situation is very bad and more health care likely won't help (it is called a Living Will since it applies when a person is still living ). Form 7. Do Not Resuscitate - these are actually 2 forms that do the serious act of immediately refusing most care, and these are short so paramedics can read them fast and they can be used outside any facility. - - - - GIVING POWER FORMS - - - - Form 8. Statutory Short Form Power Of Attorney - lets power over money, property, and other things be shared during a person's life with a trusted person like a spouse, relative, or friend so they can do things. Form 9. Authorization To Consent To Health Care For Minor - lets a parent share power over a child under 18 with someone so they have power to make decisions about health care and other things if needed. Form 10. Authorization About Bodily Remains - this form lets instructions be given and if wanted a person be named to control funeral, burial, cremation, ceremonies, and other related matters. Also provided is the Estate Planning Helpful Information form to help give family and friends information about property, money, location of accounts, debts, passwords and codes, last requests, and helpful tips.
